1. How do I get started once I have registered as a careplans.com member?
Its easy! Log in, enter your facility information, and begin building careplans!
2. How do I enter my facility information on my member account?
Log in using the member login tab on the left menu bar, or at the top of the menu screen. This will take you to a My Account Page
Select Facility Information. Once you reach the Facility screen you can edit all facility information. Under the Careplans tab on the facility screen, you have the option to select "who" the careplans are for (Residents, Patients, or Clients) dependent upon the type of facility. Check the box if you would like to include the Person option for your care plans.
3. How do I create a plan of care for a new Resident, Patient, or Client using the care plan builder?
You have three options for quick easy access to your careplan library. You can select "My Account" from the top menu bar, "My Careplan Library" on the left menu bar, or the "Careplan Builder"on the left menu bar. This will take you to your Residents Administration page.
Use the "Add New Resident" on the left menu bar or select current patient from your careplan library and begin the plan of care.
4. How do I enter my patients Demographics info and Photo?
Once a patient is selected from your careplan library, that profile will appear. On the left memu bar you can select Demographics or Photograph.
5. How do I start my plan of care after adding the demographics info?
When the patients Demographic and photo have been uploaded, from the left bar, select Plan of Care.
6. How do I add Problems, Approaches and Goals?
You have the option to create your own careplan from a blank record, Add from a facility Library, or Add from the Careplans.com Master Library. All options are located on the left menu bar under options.
To add from the Master Library, select "Add from master library" This will take you to the Careplan library. You can select any problem. By selecting the (+) sign next to each problem, you can see all the goals and approaches associated with this problem in our database. Select the goals and approaches you would like to incorporate in your current careplan and select "Import Selected Records" This will take you back to your plan of care.
By selecting any section of the careplan, you can edit all careplan information: This will include Date Onset, Date Review, Date Resolved, 1st person, Goals, Approaches, Categories, and Plan of Care notes.
7. How do I print my Patient Careplan?
Once you have imported your information and created a record, 2 options will appear in the left menu bar:
- Edit Selected Problem/Need
- Print Selected Problem/Need
Select "Print Selected Problem Need" Here you can select print to pdf or excel.
*Note: Prints best using "Landscape" orientation. Modify your printer settings using the properties option.

When selecting "Print selected problem or need", the printing options will appear. Select pdf or xls in the drop down menu, or Save the plan of care to disk.
10. How do I add users to my membership?
Please call our office and we'll be happy to help you add users to your facility.
11. How do I upgrade, renew, or cancel my membership.
Log-in and go to your "My Account" page located in the menu bar. Below Member (highlighted in blue), you will have several options listed: Member Information, Upgrade My Account, and Cancel My Account.
Upgrading your account from a basic to a full membership will give you access to all features including the NANDA-International Nursing Diagnosis Library, (NIC) the Nursing Intervention Classification, and (NOC) the Nursing Outcome classification.
12. How do I add from NANDA?
- In the Options listed in the left menu bar, select Add From NANDA-NIC/NOC
- This will bring you to our Library of NANDA-I, NIC and NOC Content. This library is alphabetically organized by NANDA-I diagnoses. The NIC Interventions and NOC Outcomes are listed under each Diagnosis as they have been linked.
- To the left of each Diagnosis Label there is a small box with a + sign. Click on the plus sign to expand the options underneath each Diagnosis.
- The NANDA-I Diagnosis library including Defining Characteristics, Related Factors, and Risk Factors depending on the type of Diagnosis.
- Make your selections by adding a green check mark to the items you want in your care plan.
- Select "Import Selected Records."
13. How do I add an additional NANDA label to my care plan?
- Once you have imported your selected records you are ready to edit your care plan. You will be taken back to the overview page. To edit the selection, click on the text of the care plan issue that you want to edit.
- A window will open to edit your careplan. This window will have a light blue bar labeled "Edit Resident Care Plan Problem"
- The top area will allow you to add/edit text to modify your statement. Since you are using a NANDA-I label, you will not be able to edit this content, but our system allows you to enter content above the NANDA label if you want to enter a description of the problem personalized to the particular patient.
- On the bottom of this page there will be several tabs: General, 1st Person, NANDA, Goals, Approaches, Rationale, Categories and Notes.
- Choose the NANDA-I tab to add additional NANDA-I diagnosis labels.
- Make your selections and press Save and Close. These additional Diagnosis will appear for this Issue Statement
14. How do I add and edit NOC content on my care plan?
- On the "Edit Resident Care Plan" there will be several tabs: General, 1st Person, NANDA, Goals, Approaches, Rationale, Categories and Notes.
- By selecting Goals, your list of Goals you have already imported will show at the bottom of the screen.
- Once you have selected the "Goals Tab" you will see options in the left menu bar to "Add New Blank Goal" or "ADD from NOC"
- To enter a new Outcome select "ADD from NOC" a new window will appear.
- Use the "Tree View" by selecting the shaded box with the + (plus sign) to expand each NOC outcome.
- If you need to edit a particular goal, simply click on the goal you want to edit.
- When you click on or select a Goal, a new window will open. This screen can be identified by the Blue bar labeled "Edit Resident Care Plan Goal".
- The top portion of this page will allow you to modify the content of the Goal. Note that NOC does not encourage the content to be modified completely, but adjustments can be made to personalize the goal to the patient.
- On the bottom of this page there will be several tabs: General, 1st Person, Indicators, Rationale, and Notes.
- You can use the Rationale tab to enter rationale for that particular Goal. This will show up on the printed care plan when you select the option to "print rationale."
- You can also modify the indicator by clicking on or selecting the tab "Indicators"
- Select the indicator you want to change or modify and a new window will open.
- Once you have completed the editing stage, select Save and Close in the upper right hand corner of the left menu bar to return to your care plan overview screen.
15. How do I add and edit NIC content on my care plan?
- On the "Edit Resident Care Plan" there will be several tabs: General, 1st Person, NANDA, Goals, Approaches, Rationale, Categories and Notes.
- By selecting Approaches, your list of Approaches you have already imported will show at the bottom of the screen.
- Once you have selected the "Approaches" Tab you will see options in the left menu bar to "Add New Approach" or "ADD from NIC"
- To enter a new Intervention, Select "ADD from NIC" a new window will appear.
- Use the "Tree View" by selecting the shaded box with the + (plus sign) to expand each NIC Intervention.
- If you need to edit a particular goal, simply click on the Approach you want to edit.
- When you click on or select an approach, a new window will open. This screen can be identified by the Blue bar labeled "Edit Resident Care Plan Approach".
- The top portion of this page will allow you to modify the content of the Approach. Note that NIC does not encourage the content to be modified completely, but adjustments can be made to personalize the Approach to the patient.
- On the bottom of this page there will be several tabs: General, 1st Person, Disciplines, Rationale, and Notes.
- You can use the Rationale tab to enter rationale for that particular Goal. This will show up on the printed care plan when you select the option to "print rationale."
- You can select the Discipline you want to show on your care plan by checking the box next to the appropriate Discipline.
