Head, Eyes, Ears, Nose, & Throat Assessment

Normal HEENT Assessment Findings

Head

  • Normocephalic and atraumatic
  • No tenderness, palpable masses, depressions, or scarring
  • Hair evenly distributed with normal texture

Eyes

  • Visual acuity intact
  • Conjunctivae clear, no redness, exudates, or hemorrhage
  • Sclera white, non-icteric
  • Extraocular movements intact, symmetrical
  • Pupils equal, round, reactive to light and accommodation (PERRLA)
  • No nystagmus

Ears

  • Pinna, tragus, and ear canal non-tender, no swelling
  • Ear canal clear, no discharge or foreign bodies
  • Tympanic membrane pearly white or light gray, intact, visible cone of light
  • Hearing intact, normal response to whispered voice or conversational speech

Nose

  • Nasal mucosa pink, moist, no lesions or abnormal discharge
  • Nasal septum midline, no deviation
  • Nares patent bilaterally

Throat/Mouth

  • Oral mucosa pink, moist, no lesions
  • Dentition intact, no significant decay
  • Tongue normal, no lesions, symmetrical movement
  • No buccal nodules or lesions
  • Pharynx without redness, swelling, tonsillar enlargement, or exudates
  • No lymphadenopathy

Neck

  • Trachea midline
  • Thyroid gland non-enlarged, no masses, nodules, or tenderness
